Incident investigation and root cause analysis — Sev1–3 RCA with regulatory flagging.

Watch is an incident investigation and root cause analysis module. When a production incident or operational event occurs — a Sev1 API outage, BACS settlement failure, fraud spike or regulatory notification — Watch structures the investigation: timeline, probable cause, evidence, counter-evidence, regulatory reporting obligations, and drafts a defensible root cause analysis. Watch is not a regulatory horizon scanner. It investigates operational incidents.

Step by step

1
Incident submitted
The user submits an incident: title, description, severity classification (Sev1, Sev2 or Sev3), and any initial evidence.
2
Timeline constructed
Watch assembles a chronological timeline of events relevant to the incident, drawing on your indexed data and the submitted evidence.
3
Cause analysis drafted
Probable cause identified with supporting evidence. Counter-evidence surfaced. Alternative hypotheses assessed.
4
Regulatory obligations flagged
Watch checks the incident type against FCA operational resilience requirements (PS21/3) and DORA reporting obligations. Flags what must be reported and by when.
5
RCA delivered
A structured, defensible root cause analysis is produced, ready for internal review, regulator submission or board reporting.
